The episode begins with a flashback that shows us what happened the day Rick was shot: Lori is talking to a friend saying she thinks she still loves rick when Shane arrives at Carl's school and tells Lori about the shooting.  

Following the flahsback, "Bloodletting" picks up right where it ended last week. Rick's son Carl has been shot, an apparent hunting accident shot by another zombie survivor named Otis.

Otis leads Rick and Shane back to a farm where a warm-hearted survivor named Hershel lives with his family. Hershel's first question: "Was he bit?" On confirmation that Carl hasn't been infected, Hershel marshals his daughters, Patricia and Maggie, as well as Otis to assemble his medical gear and begin treatment.

Back at the RV, Dale's hard at work under the hood, prying spare parts from the broken-down cars. T-Dog seems worried, so Dale asks how he's feeling. T-Dog ignores the question, so Dale pushes the issue, and the news isn't good: T-Dog's wound has become severely infected. Dale tells T-Dog he might die from blood poisoning (T-Dog finds this morbidly funny) and tells his fellow survivor that they'll have to scavenge for antibiotics in the abandoned car graveyard.

Shane keeps Rick on track as Hershel works to save Carl's life. Before Shane can shake Rick from his emotional duress, Hershel asks Rick to provide blood. When Rick goes into the makeshift OR, Carl's screaming his head off; the post-apocalypse is not exactly brimming with sedatives. Rick gets his blood work done as Shane comforts Carl, who promptly passes out. Hershel: "One down... five to go."

Rick wants to locate Lori and bring her back, but Hershel insists that he stay within 50 feet of Carl's bed. And there's worse news: To save Carl, Hershel needs some advanced equipment--most importantly, a respirator. Otis knows just the place to scrounge for equipment: A nearby high school where FEMA set up an emergency triage facility. Rick can't leave, so Shane heads up the expedition to the high school. Otis volunteers to guide Shane to the facility to help make amends for his costly mistake. Maggie heads out on horseback to locate Lori.

The rest of the group has run into some trouble in the woods: In what I consider the best zombie attack of the season, if not the series, Andrea falls victim to a stealthy Walker who seems to come from nowhere. Knocked prone, Andrea struggles helplessly before Maggie arrives and takes out the Walker. She brings Lori back to Hershel's farm; the rest of the group heads back to the RV, where T-Dog's fading fast.

Back at the farm we have a couple brief but incredibly compelling revelations. First off, a conversation between Hershel and Rick reveals the patriarch's interesting views on the zombie epidemic: Mankind always gets its butt kicked in the early days of historical plagues, but bounces back soon after. Hershel views the zombie plague as merely the next in a series of balance-restoring countermeasures by nature itself. Second, and more pressing, we discover that Hershel's not especially experienced in the type of surgery he's about to perform, pending the arrival of supplies: Before the apocalypse, he was a vet. Not a combat medic--a veterinarian. Lori is floored.

Back on the highway, the team is taking the RV and leaving--Darryl and Dale are staying behind to leave supplies and a sign for Sophia. Glenn's ordered to bring T-Dog to the farm to find antibiotics, but Darryl has a better plan: He busts out his brother's stash, which contains all manner of illegal drugs along with some seriously nasty antibiotics.

Shane and Otis make it out to the high school, which is crawling with Walkers. As night falls, they prepare for a dash across the parking lot to the medical trailer. A nearby police cruiser provides some road flares that the survivors use as distractions to make their way to the trailer unscathed. Getting in is easy, but getting out is a nightmare: Running from the herd, Shane and Otis end up stranded behind a foldable fence with a flimsy pen separating them from disaster. The pin begins to give as... the episode ends.
