Determined to save her family at any cost, Gwen takes Jack captive to deliver him to the kidnappers.
A flashback to 1927, unites Jack and a young Italian immigrant named Angelo.

In the immigrant inspection station on Ellis Island, New York City, USA 1927, an inspector calls out for Jack Harkness. A guy steps up and receives a visa, but before he can enter the USA, Jack shows up and stops him. As Jack pins him down, they look deep into each other's eyes. Guards step in and Jack explain he's the rightful owner of the visa while the other guy is taken away to go back home.

While he is waiting for the next boat to Manhattan, Jack visits the guy in his holding cell who is waiting for a ship back to Italy. He reveals that he discovered Jack's visa to be a forgery, and Jack explains that he is on a kind of unofficial official visit. Jack wants to know why his English is so good and where he comes from, and the guy explains that they had a schoolmaster in his village where he went every day because he always dreamt of going to America. Jack asks for the guy's name who introduces himself as Angelo Colasanto. Then Jack takes his visa and uses his vortex manipulator to change the name to Angelo Colasanto. It scares Angelo who crosses himself and Jack explains it's not magic, but technology. Angelo wants to know how it works, but Jack calls out instead and announces that they found Angelo's papers and that they can go to Manhattan.

Back in the present, Gwen tries to call her mother and Rhys after the people who hacked her I-5 feed claimed they took her family, but the calls keep going to voicemail as the two are being watched in their home. They tell Gwen to keep the lenses in and add that they are watching.

At the Torchwood office, Esther checks the youtube video with Vera's death which has reached over five million views. She tries to comfort Rex who turns her down and explains he didn't even know Vera well. Gwen storms in and calls out for Jack. Esther asks about news from Gwen's father, but Gwen keeps it short and asks Jack to follow her to her car because she wants to show him something from home. When Jack leans into her car, she tasers him.

Back in 1927, Jack and Angelo rent a room in Little Italy, New York City. Jack advises Angelo to save his money for bad times to come. Then he adds it will get better, and then worse again. Angelo looks out of the window and argues there's not much of a view though there is a beautiful woman on a balcony on the other side of the street. And as Jack describes to Angelo what he would like to do to that woman, the sexual tension rises until they give in and have sex.

After sleeping together, Angelo is surprised to learn that Jack wants him to stay. Though he slept with men before, he never bonded with them.

Jack regains consciousness and finds his hands and feet tied. Gwen explains that someone took her family and that they are using her lenses. He asks to let him talk to them. He wants them to let Gwen's family go and offers to go to them alone, but they don't answer. Gwen holds him responsible for what has happened to her family and what has happened to the world. She asks him to think about what he did, but he doesn't know.

Back in 1927, Jack and Angelo watch a wedding in a church. Jack mentions that he loves weddings, but Angelo doesn't believe in them. Jack explains that he acknowledges the courage it takes to make that promise. He wants to know why Angelo isn't praying, and Angelo believes no one is listening because of what he is. Then the padre Jack needs to speak to appears. The padre sells them a sample of his sacramental wine.

When Jack and Angelo come home, three men already await and grab them.

Back in the present, Jack asks Gwen if they said anything they could use to identify the people who want to take Jack. He claims he could use his wrist strap to find Gwen's daughter Anwen using Gwen's blood, but she calls his bluff as well as the people who watch them through the lenses as they add "he always lies."

Back in 1927, the guys take Jack and Angelo to a mobster called Salvatore Maranzano who is upset about them trying to undercut his prizes. However, Jack discovers that Maranzano needs them for a job or they's already be dead. He asks Maranzano to give them a try who reveals that he needs a box from one warehouse relocated to another and makes them swear not to look in the box.

Back in their room, Jack starts packing Angelo's clothes and tells he needs to go because the people are too dangerous. Angelo realizes Jack arranged to meet Maranzano all along and wants to stay. Jack tells him he knows that Angelo is praying for forgiveness everytime they had sex and adds if it's impossible for two men to find love, then Angelo can't stay with him. However, Angelo assures him Jack's secrets can't change his life more than it already has, and that he feels guilty, but stayed with Jack anyway. Jack changes his mind and starts unpacking Angelo's stuff. He mentions his friend, the Doctor who chooses to explore the world with a companion and adds that it does look nice.

The two enter the warehouse where Jack opens the box. Inside they find an alien, a parasitic species of Brainspawn which causes the infected to go insane within a couple of years. Jack explains that the parasite is destined for Franklin Roosevelt to change the outcome of World War II and to destroy the future. Maranzano has been used by someone called the Trickster's Brigade who turn order into chaos and feeding off the results. They kill the parasite and start running when an alarm goes off. Jack helps Angelo climb a fence, but before Angelo can pull Jack up, the police shoots Jack and Angelo watches Jack die as the last bullet hits his forehead. Soon later, they arrest Angelo, but before they come back to take Jack's body away, his wounds heal, he wakes up and escapes.

Back in Gwen's car, she tells Jack that she caused this. She reveals that she loved being part of Torchwood though she knew it was toxic because it made her feel special and important. Now she is ready to sacrifice Jack for the sake of her daughter. In return, Jack assures her that he is determined to hold on to his mortality even if he had to rip her skin from her skull.

In 1928, Angelo leaves the Sing Sing Prison in New York, USA, and is shocked to find Jack waiting for him. Jack tries to explain that he was just playing dead, but Angelo saw that he got shot in the head. He takes Angelo's hand to feel his heart beat and adds that this is the best of his secrets.

Jack rented the same room to continue their relationship. When they lie on the bed kissing, Angelo flashes back to the day Jack got shot. He takes a knife and stabs Jack, calling him the devil. Jack tries to fight him off, but in his rage, Angelo kills Jack.

When Jack comes back to life, Angelo, the butcher and his wife who rented the room to him are witnessing his resurrection. The butcher is as shocked as Angelo and kills Jack again.

They take him to the butcher's shop where the locals repeatedly torture and kill him. Angelo soon regrets what he did to Jack, but can't help him. Later, Jack awakes when three men discuss paying the butcher off and sealing a partnership with a triangle-shaped three-way handshake.

In Mesa, California, USA, Gwen and Jack have reached their destiny. Gwen asks Jack about the most beautiful thing he ever saw and he tells her about a tiny firebird, literally made of fire, blazing different color, and only living for a minute. Gwen starts crying and asks Jack if it's a consolation that he lived so many lifetimes. However, he replies it is not enough and that he doesn't want to die and Gwen says she's sorry.

Back in 1928, Angelo who bought some inattention from Jack's guard, frees Jack and helps him escape. Jack wants to know who the three men were, but Angelo doesn't know.

Angelo wants them to catch a train towards the West Coast, but Jack climbs fire escape stairs to a roof where he hid his coat and pistol. He tells Angelo that he is going alone since it's time to move on. Angelo apologizes for what happened and explains that he was terrified. Jack tells Angelo that once something happened and time changed him to a fixed point which made him immortal. He adds that he lived through part of the Earth's past and part of its future. Angelo wants to know if they are together in the future, but Jack tells him they can't since Angelo will die one day. Angelo answers to live in the moment and begs Jack not to leave him. Jack apologizes and tells Angelo that the story with men like him always end the same, that they always kill him. Then he jumps off the roof. Angelo runs down the stairs, but when he reaches the ground, Jack is already gone.

The car for Jack arrives and two armed man and a woman get out. The woman is pleased to see they followed their instruction and greets Jack, the last mortal man. Suddenly someone shoots near the feet of one of the guards, then the red dot of the laser appears on the woman's chest. It's Esther and Rex aiming at them. A flashback shows how Esther discovered the I-5s cache and tracked the signal.

Gwen and Jack realize Esther and Rex are backing them up. Gwen unties Jack and they disarm the guards. When Esther and Rex see their hands up, they give a signal for a SWAT team to free Gwen's family.

Rex and Esther join Gwen and Jack. Esther has good news and tells Gwen that her family is safe. Jack asks the woman to tell them who she is before he let's Gwen deal with her, but the woman explains nothing has changed and that he is still coming with her. He wants to know why, and she tells him she can take him to the man who knows about the origin of the miracle. She adds that Angelo Colasanto is waiting for him and that he has waited for a long time.
